Day (United States of America)The Case Management Department provides case management services to patients in the Emergency Department and on the inpatient units through specially trained registered nurses.
These nurses work collaboratively with the patient, family, patient care team, health insurer and healthcare providers in the community to develop an individualized discharge plan.
Our compassionate team is committed to supporting patients in receiving the highest quality care in the most appropriate setting after discharge from the hospital.
This position is per diem, with various hours and shifts available.

Job DescriptionPOSITION FUNCTION:
Performs duties appropriate to a Medical Social Worker and maintains confidentiality in this process as required by law and Milton Hospital.
Offers assessment, counseling, and discharge planning services to Milton Hospital patients, families, or significant others who are in need of Social Service Department intervention.
May assume necessary administrative and supervisory responsibilities in the absence of the Director of the Case Management Department.

Responsibilities:
Under the general supervision of the Director of Case Management, assumes the following
Responsibilities:
Participates in the daily screening and identification of high-risk patients who may need social work or discharge planning services.
Attends staff meetings with multidisciplinary rounds for high-risk screening, sharing of clinical expertise with the medical team, and for case referral.
Works in collaboration with the medical staff, the hospital's multidisciplinary team, and the with community facilities to coordinate appropriate discharge plans for patients served.
Carries a full caseload of patients needing social work and/or discharge planning services.
Evaluates the psychosocial, medical, economical, and cultural needs of all assigned patients, families, or significant others needing social work intervention.
Provides short-term psychosocial counseling and crisis intervention for patients, families, or significant others who are experiencing major life adjustments due to illness, treatment, or prognosis.
Arranges for needed medical and/or psychiatric post-hospital services.
Provides consultation/education for the Department's Case Management Nurses when patients need comprehensive psychosocial assessments, counseling, or when assistance is needed in making arrangements for psychiatric care, substance abuse rehabilitation, or in cases of domestic violence, child, elder, or disabled person abuse, or in other urgent psychosocial situations.
Coordinates planning with Case Management Nurse when making arrangements for nursing home, rehabilitation or chronic disease hospital placement.
Refers patients to Case Management who need medical assessments and/or level of care determinations.
Helps clarify with patients, families, or significant others the process of utilization review, including the circumstances around the issuance of the Notice of Noncoverage.
Collaborates with the Patient Accounts Department in processing Medicaid applications.
Assists patients, families, or significant others with same.
Assists Case Management Nurse with Medicaid issues as needed.
Complies with the Departments' documentation standards for the medical record and the internal case files.
Provides accurate statistical date to the Social Service/Case Management Department Secretary.
May assume necessary administrative and supervisory responsibilities in the absence of the Director of Case Management.
Provides consultation and education for hospital staff and the community, as appropriate.
Provides information and referral services to the community.
May participate on community planning committees, in task-force groups, or conduct community support groups.
Collaborates with other community heath care and facilities, and evaluates their service delivery.
Assists with quality assurance activities and participates on hospital planning committees when directed.
Attends professional conferences and workshops to keep abreast of current clinical interventions, regulations, and resources pertinent to the delivery of social work and discharge planning services, as well as to maintain licensure.
Performs all duties in accordance with the laws, rules, and regulations set forth by appropriate regulatory and government agencies, and in accordance with established departmental and hospital policies and procedures to promote the safety and health of the worker, co-workers, patients, and visitor in Milton Hospital.
May be required to perform similar or related duties as may be necessary on an emergency, temporary or limited basis as assigned.
All job functions are subject to change at the discretion of Hospital AdministrationQUALIFICATIONS;Must have an MSW degree with two years post-MSW experience, preferably in a health care setting, OR a BSW degree with at least three years experience in an acute hospital settingMA Licensure is required at the appropriate levelFLSA Status:
